Summary

Formal SEP for the ToolFailure extension prototyped in #3312, based on
discussion #2930. Placeholder SEP number (0000) per SEP-1850 -- will rename
the file to match this PR's number once assigned, and update the header
accordingly.

@SamMorrowDrums would you be open to sponsoring this SEP per SEP-1850? It builds on the tool-annotations work (idempotentHint) to add structured failure classification for CallToolResult, and the schema-level prototype is already up in #3312. Happy to make any changes you'd want before requesting review.
